Single Incision Gastric Banding

This is a new advanced laparoscopic technique where the Adjustable Gastric Band is placed through a single incision instead of five small incisions used for traditional laparoscopic gastric banding surgery. The surgeon uses a single 7-centimeter horizontal incision as is usually done for the access port placement in the epigastrium (below your chest bone). Through this incision 3 miniature trocars (sleeves through which miniature instruments are introduced) are placed. The miniature instruments as well as a 5mm scope are used to place the band around the upper part of the stomach in the usual way. The band is stitched in place in the usual manner. The difference is in the size of the incision and the instruments used which reduce scarring and accelerate healing. Fewer scars are better both cosmetically and for pain control.

This novel technique is used in NOTES surgery which means: Natural Orifice Trans Endoscopic Surgery. This is when the surgeon uses the mouth or the vagina as means to remove appendix or gall bladder without any abdominal incisions. NOTES cannot be used with a prosthesis like the lapband because of contamination issues but the special instruments can be used to place it through one single incision reducing the scarring and speeding the recovery for well selected young and lower BMI patients which are the ones most benefited by this technique.
